Team Registration

Team registration is due June 10, 2011. Any team packets turned in after June 10th will be charged a $100.00 late fee. No packets will be accepted after June 30th. Academy registration is due 1 month later. Please make sure you follow these instructions, and if you have any questions or need further assistance, please contact the Strikers club registrar.

1. All players will be required to register and pay online.
1. Credit cards will not be charged until a team has been accepted and activated.
2. All pictures and birth certificates will need to be uploaded to the ADG system in order to complete the registration process.
1. New players to UYSA must turn in a hard copy of their birth certificate and player registration form to the Strikers Registrar.
3. Coaches must turn in the following to the Strikers Registrar before their team can be formed:
   1. Complete player roster with coach’s signature.
   2. Volunteer Disclosure for ALL coaches and team administrators.
   3. Copy of Coaches License or License Waiver for new coaches that don’t have their license.  (THIS WAIVER IS ONLY GOOD FOR ONE YEAR)
   4. Alignment form. Both STATE and DISTRICT teams are required to turn in an alignment form.
   5. Coaches code of conduct for coaches and all assistant coaches
   6. Copy of birth certificates for all new players
4. Coaches will be given an AUTO ASSIGN CODE from Strikers Registrar once all the information above has been turned in and the team has been formed. The AUTO ASSIGN CODE will need to be given to the parents on your team in order to complete the player registration process.

   IMPORTANT: Players will only be allowed to register for ONE team. So it is imperative that they are committed to your team before you give them your team’s Auto Assign Code.

5. Once a player has entered the Auto Assign Code for your team they WILL NOT be allowed to transfer.  NO transfers will be allowed until November 15th.

2. Current photo (passport type photo – face only) of the player
3. Birth certificate (only new players)
